Terms and Conditions
Privileges in Effect, membership privileges will become immediately in effect upon the date of the Welcome Packet being received.
Termination. NCM Member may terminate this Enrollment Agreement at any time and for any reason upon thirty (30) days advanced written notice to New Care Medical (1217 S. East Ave. Sarasota, Fl 34239). Upon termination by the Member for any reason, there shall be no refund of the Membership fee. In the event this Agreement is terminated by either party, Member acknowledges that the NCM Membership Program will no longer be in effect and that Member will be responsible for obtaining an alternative primary care physician for medical care. In addition, in the event of such termination, Member will not be able to submit a claim to Medicare or other insurance for any of the medical care and services provided by the NCM Participating Provider pursuant to the terms of this Agreement.
NCM or NCM Participating Providers DO NOT Bill Insurance. NCM Membership is a self-pay program and does not bill insurance. This is regardless of whether some of the services NCM or NCMPP provides are covered by insurance. Patient acknowledges that NCM or NCMPP will not file or accept an insurance claim as payment for patient care, and the NCM or NCMPP makes no representations or promises regarding whether their services are covered by insurance or reimbursable through a health care spending account or other health care product. Notwithstanding the foregoing, the NCM or NCMPP will assist Member by providing any reasonable documentation which may be necessary for Member to seek reimbursement from an insurer, health care spending account, or other health care product. Member acknowledges that the NCM or NCMPP may be considered an out of network provider for Member's third party insurer, and Member assumes sole responsibility for determining the referral requirements of Member's third party insurer.
